Understanding HPV: What Gardasil Protects Against

Human Papillomavirus (HPV) is the most common sexually transmitted infection worldwide. How Does Gardasil Work? (Mechanism of Action)

Gardasil is a recombinant, non-infectious (no live virus) subunit vaccine. It works through a 4-step immunological process:

Virus-Like Particle (VLP) Injection
Each dose contains L1 proteins (outer coat proteins) from HPV Types 6, 11, 16, and 18 that self-assemble into hollow VLPs – these mimic the HPV capsid without any viral DNA.

Adjuvant Activation (AAHS)
Amorphous Aluminium Hydroxyphosphate Sulfate (AAHS) acts as a powerful adjuvant – it amplifies the immune response, causing dendritic cells and macrophages to engulf VLPs and present them to T-cells.

Antibody Production (Seroconversion)
B-lymphocytes produce high titres of HPV type-specific neutralising antibodies (IgG). Memory B-cells and T-cells are created for long-term protection.

Protection at Infection Site
High antibody titres at mucosal surfaces (cervix, vagina, vulva, anus) neutralise actual HPV virus on contact – preventing the virus from infecting epithelial cells and establishing persistent infection that could lead to cancer.

❓ Frequently Asked Questions About Gardasil

Is HPV vaccine effective if already sexually active?
Yes. Even if you are already sexually active or have been infected with one or more HPV types, Gardasil can still protect against the strains you have not yet been exposed to. The vaccine works best before any HPV exposure (ideally age 9–12), but the IAP recommends catch-up vaccination up to age 26.
Can Gardasil 9 be given after Gardasil 4?
Yes. Gardasil 9 can be given as a series even if you previously received Gardasil 4. It provides additional protection against 5 extra HPV strains (Types 31, 33, 45, 52, 58) that are responsible for another ~15–20% of cervical cancers. Consult your doctor about the dosing interval.
